Smile Please Translate from English to Vietnamese

Hi, I have a friend who I am getting to know and I'd like to have a few questions translated to Tieng Viet so I can ask her. Thank you.

    What is your full name?

When was the last time you had a boyfriend?

What do you like to do for fun in your free time?

I really like Vietnamese food. Are you a good cook?

If I go back to Vietnam in a few months, would you be able to take a few days off work to spend time with me?

Miss Ivy - Thank you for your help.

Họ và tên của em là gì vậy?/ Tên đầy đủ của em là gì vậy?
Do these both mean the same thing? Which one would be best to use? I just want to know her full name.

Take things slowly, do not rush into a relationship. It should be a thing that lasts a long time.
