Are these term for medical record for a hospital?

These are medical terms but not for the hospital, they were just for the private practices which I am volunteer helping the students and parents on the day of the visit. This private practices clinic offer a free dental cleaning for the refugee students. As I've read the whole documentations they sent me and those words are the words I am concern and will be stuck with if interpret for them. Vietnamese is not my primary language after all, so if anyone could help me with these I would greatly appreciated.
